At the Onen’to:kon Healing Lodge, we strive to make access to our treatment programs as simple and straightforward as possible. Our Application Kit is available as a fillable PDF that can be completed electronically on your computer, or printed and filled out by hand.

Completed applications may be submitted by email, fax, or mail using the contact information provided below.
