At the Onentokon Healing Lodge, we strive to make access to our treatment programs as straightforward as possible. We have made the Application Kit fillable, so you may complete it out on your computer, or, print it and fill it out by hand. 

You may return the completed form to us in one of several ways:

1. Print out the form, fill it in by hand and return it to us by fax or mail  as using the information below.
